This is so much a cultural thing anyone is going to have to make their own decision. Where I work, I have noted social mores vary within even a few tens of kilometres (city/smaller towns).
You also want to do what you are going to feel comfortable with yourself.
My own practice is "no", though frequently someone helps or passes through. But then, as I think I've commented previously on this list, not even gynaecologists have chaperones here.
My one firm note would be the danger of establishing any (personal or group) 'norm'. Stating one always does (or one always should) do 'so and so', sets a level that leaves you high and dry if you ever depart from it.
Whatever you decide, hope it's comfortable for you, the model (and the assistant).
